Entropion

👁️ Entropion

🔍 Definition

Entropion is a condition where the eyelid margin turns inward, causing the eyelashes and skin to rub against the surface of the eye (cornea and conjunctiva). It most commonly affects the lower eyelid.

🧬 Types of Entropion

TypeCauseCommon Age Group
InvolutionalAge-related degeneration of eyelid tissuesElderly
CicatricialScarring of the conjunctiva causing lid to turn inAny age (often due to injury or disease)
SpasticEyelid muscle spasm, often due to irritation or inflammationAll ages
CongenitalRare; due to improper development of eyelid structuresInfants

🧠 Etiology (Causes)

  • Aging (most common): weakened lower eyelid retractors

  • Chronic eye inflammation: e.g., trachoma, Stevens-Johnson syndrome

  • Chemical burns

  • Trauma or surgery causing conjunctival scarring

  • Infections: trachoma, herpes zoster ophthalmicus

  • Congenital malformation

👀 Clinical Features

Symptom / SignDescription
Foreign body sensationDue to lashes rubbing against cornea
Redness & tearingReflex tearing and irritation
PhotophobiaSensitivity to light due to corneal irritation
Blurred visionIf cornea becomes damaged
TrichiasisInward turning of eyelashes
Conjunctival injectionRedness due to irritation
Corneal erosions/ulcersMay occur in advanced cases

🔍 Diagnosis

  • Clinical exam – observe lid position and lashes

  • Fluorescein stain – detects corneal epithelial defects

  • Snap-back & distraction tests – assess eyelid laxity

💊 Treatment

🔹 Conservative (temporary)

MethodWhen Used
Lubricating dropsTo protect cornea
Eyelid tapingTemporarily turns lid outward
Botulinum toxin injectionInto orbicularis to relax muscles
Bandage contact lensProtects the cornea

🔹 Surgical (definitive)

ProcedureType of Entropion
Lid tightening proceduresInvolutional
Everting sutures (Quickert sutures)Involutional / temporary correction
Mucous membrane graftingCicatricial
Scar release with skin graftCicatricial

⚠️ Complications

  • Corneal abrasions

  • Recurrent eye infections

  • Corneal ulceration → scarring → vision loss

  • Chronic conjunctivitis
