Lagophthalmos

๐Ÿ‘️ Lagophthalmos

๐Ÿ” Definition

Lagophthalmos is the inability to fully close the eyelids, leading to exposure of the cornea and conjunctiva. This can cause dryness, irritation, and even corneal ulceration if severe.

๐Ÿง  The term comes from the Greek word lagos, meaning hare — inspired by how hares sleep with their eyes partially open!

๐Ÿงฌ Causes of Lagophthalmos

TypeCommon Causes
ParalyticFacial nerve (CN VII) palsy (e.g., Bell’s palsy, trauma, tumors)
NocturnalEyelids don't close fully during sleep (idiopathic or post-surgery)
CicatricialEyelid scarring (burns, trauma, Stevens-Johnson syndrome)
MechanicalEyelid retraction from thyroid eye disease, tumors
Post-surgicalOvercorrection from blepharoplasty (cosmetic eyelid surgery)

๐Ÿ‘€ Clinical Features

FeatureDescription
Inability to close eyelids completelyEspecially noticeable during blinking or sleep
Lag signGap between eyelids when attempting closure
Dryness & irritationDue to corneal exposure
Tearing (paradoxical)From reflex lacrimation
Redness, foreign body sensationOften from exposure keratitis
Exposure keratopathyPunctate epithelial defects, ulcers, infection

๐Ÿฉบ Diagnosis

  • Clinical observation – ask patient to close eyes slowly and gently

  • Check Bell’s phenomenon – protective upward rotation of eyeball when eyelids close

  • Slit-lamp exam – assess corneal surface

  • Fluorescein stain – highlights corneal epithelial damage

  • Facial nerve testing – especially if palsy is suspected

๐Ÿ’Š Treatment

๐Ÿ”น Conservative Management (mild to moderate)

MethodPurpose
Lubricating eye drops/gelMaintain moisture during day
Lubricating ointment at nightPrevent drying overnight
Moisture goggles / patchingProtect eye during sleep
Taping eyelids closedTemporary solution for nighttime lagophthalmos

๐Ÿ”น Surgical Management (severe or persistent)

ProcedureIndication
TarsorrhaphyTemporary or permanent partial lid closure
Gold/platinum weight implant in upper eyelidHelps eyelid close by gravity (for CN VII palsy)
Skin grafts / lid reconstructionFor cicatricial lagophthalmos
Lower eyelid tighteningHelps protect cornea by elevating lid

⚠️ Complications

  • Exposure keratitis

  • Corneal ulcer

  • Scarring and vision loss

  • Chronic conjunctivitis

๐Ÿง  Mnemonic: “LAG = Lid Ain’t Going (all the way)”
