Symblepharon

๐Ÿ‘️ Symblepharon

๐Ÿ” Definition

Symblepharon is the adhesion (scar-like attachment) between the palpebral conjunctiva (lining of the eyelid) and the bulbar conjunctiva (covering the eyeball), leading to a restriction of eye movement and eyelid mobility.

๐Ÿ“Œ It does not involve the cornea directly but may limit vision by distorting eyelid and conjunctival anatomy.

๐Ÿงฌ Etiology (Causes)

CategoryCommon Examples
Cicatrizing conjunctivitisStevens-Johnson syndrome, ocular cicatricial pemphigoid
Chemical burnsEspecially alkali burns, which penetrate tissues deeply
TraumaDirect injury to conjunctiva or burns
Post-surgicalAfter conjunctival surgeries without proper precautions
InfectionsSevere trachoma (late stage)
Radiation exposureRare but can cause conjunctival fibrosis

๐Ÿ‘€ Clinical Features

Symptom / SignDescription
Reduced eye motilityAdhesions restrict free movement of the eyeball
Foreign body sensationDue to conjunctival scarring and distortion
Redness, irritationFrom dry spots and poor tear film distribution
Cosmetic deformityVisible web-like adhesions in severe cases
KeratopathyFrom reduced blinking and lubrication
Shallow or obliterated fornixLoss of normal conjunctival space

๐Ÿ” Types of Symblepharon

TypeAdhesion Between
AnteriorLid margin to bulbar conjunctiva
PosteriorPalpebral conjunctiva to bulbar conjunctiva deeper in fornix
TotalExtensive adhesion involving large area
PartialLocalized band(s) of adhesion

๐Ÿงช Diagnosis

  • Clinical examination – evert eyelid to inspect conjunctiva and fornices

  • Slit-lamp – assess adhesions, conjunctival scarring

  • Schirmer’s test / tear film assessment – if dryness is present

๐Ÿ’Š Treatment

๐Ÿ”น Prevention (important after burns or surgery)

MethodPurpose
Frequent ocular lubricationPrevent drying and adhesion formation
Use of glass rod or symblepharon ringMaintains fornix depth
Early eyelid stretching / massageTo prevent scar contraction

๐Ÿ”น Surgical Management

ProcedureIndication
Symblepharon lysisSurgical cutting of adhesion
Amniotic membrane graft (AMG)Covers defect post-lysis, reduces recurrence
Mucous membrane graft (e.g., buccal)For extensive cases or total symblepharon
Limbal stem cell transplantationIf limbal deficiency is contributing
Symblepharon ring insertionKeeps fornix formed post-surgery

⚠️ Complications

  • Recurrence (common without proper post-op care)

  • Dry eye

  • Reduced ocular motility

  • Cosmetic deformity

  • Difficulty wearing contact lenses

๐Ÿง  Mnemonic: "Sym-bond = Symblepharon"

➤ "Sym" = together, "blepharon" = eyelid → structures stick together abnormally.
