Trichiasis

๐Ÿ‘️ Trichiasis

๐Ÿ” Definition

Trichiasis is a condition where eyelashes grow inward, toward the eyeball, and rub against the cornea and conjunctiva, causing irritation, inflammation, and possibly corneal damage.

✅ The eyelid margin is in a normal position — unlike entropion (where the lid itself turns inward).

๐Ÿงฌ Etiology (Causes)

CategoryExamples
IdiopathicCommon in elderly without clear cause
InfectiousTrachoma (Chlamydia trachomatis), Herpes zoster
InflammatoryBlepharitis, Stevens-Johnson syndrome
AutoimmuneOcular cicatricial pemphigoid
Trauma/SurgeryScarring from burns, lid injuries, previous surgery
CongenitalRarely present from birth

๐Ÿ‘€ Clinical Features

Sign / SymptomDescription
Foreign body sensation“Something in the eye” feeling
Redness (conjunctival injection)Due to irritation from lashes
Tearing (epiphora)Reflex tearing to flush irritant
PhotophobiaLight sensitivity from corneal irritation
Blurred visionIf corneal surface is scratched or damaged
Corneal epithelial defectsSeen on fluorescein stain exam

๐Ÿงช Diagnosis

  • Slit-lamp exam – shows misdirected lashes rubbing on cornea

  • Fluorescein staining – highlights abrasions or ulcers

  • Eversion of lid – to rule out entropion or embedded foreign body

  • History – trauma, surgery, infection

๐Ÿ’Š Treatment

๐Ÿ”น Temporary Relief

MethodUse
Lubricant eye drops/ointmentReduce friction and protect cornea
Epilation (lash removal with forceps)Quick fix, but lashes regrow in weeks
Bandage contact lensProtects cornea from lash trauma

๐Ÿ”น Definitive / Recurrent Cases

ProcedureDescription
ElectrolysisElectric current destroys lash follicles
CryotherapyFreezing lash follicles to destroy them
Radiofrequency ablationThermal destruction of follicles
Lid margin rotation surgeryFor severe or scarring-associated cases

⚠️ Complications

  • Corneal ulcer

  • Corneal scarring

  • Vision loss

  • Chronic conjunctivitis

๐Ÿ” Differential Diagnosis

ConditionKey Feature
EntropionEyelid turns inward (plus trichiasis)
DistichiasisExtra row of lashes from meibomian glands
PseudotrichiasisLashes touch cornea due to entropion

๐Ÿง  Mnemonic: “Tricky Lashes” = Trichiasis

  • Tricky lashes grow the wrong way → scratch the cornea.
